Community Support Services, trading as Eleven Sisters Health Care to you.

Our Services:

We provide qualified Nurses, Health Care Assistants and Support workers in a

flexible and dependable manner over 24 hours period and on a daily basis.

Our Duties:

The care we provide ranges from assistance with Nursing Care, Personal, Practical

and Sitting service.

Our aim

Is to maintain independence whilst ensuring dignity, privacy, respect and the well being of our Clients.

·         We provide specialized care and support for children and adults with complex

needs, including those with learning difficulties and challenging behaviour.

·         We are involved with hospital at home treatment of children requiring

ventilation, tracheostomy care and its management, epilepsy care and management,

along with Gastrostomy care requiring PEG Feed.We care for all children with Global developmental need and their families.

·         We also provide support for children and family requiring palliative care treatment whom are terminally ill.

·         We provide care and support for children and their family who require respite

care and school Nurses to both Special Needs and Mainstream schools.

Our Nurses specialize in caring for children with nutritional assistance, catheter care,

management of tracheostomy, epilepsy in children and young adults and carry out

medication management.

Our nurses and care workers undergo regular training including mandatory and

practical knowledge to perform their roles effectively.

My name is Esther Mensah and I am the Registered Provider. My qualifications are as follows:-

1.        Registered General Nurse with 37 years’ experience

2.        Registered Sick Children Nurse since 1996

3.        Qualified Midwife with 38 years’ experience

We offer our services to everyone irrespective of race, ethnic background, religion,

marital status, gender, sexual orientation, political affiliation or mental health status and under the Equality and Diversity Policy.

. We have capacity for both Nurses, Paediatrics and General Nurses, Health Care

Assistants and Support Workers to cover clients in the locations of your borough and beyond.

Our Policies and Procedures:

We have a robust up-to-date Policies and Procedures in place to ensure that the

services we deliver are accurate, consistent and responsive to our clients, their

families and our regulators and this enable us to deliver a long term success on our

promises to our customers.

Registration and Regulatory Body:

Eleven Sisters Community Support Services is registered and regulated by the Care

Quality Commission (CQC) for effective service delivery and to ensure that the

service we provide meets the government standards of quality and safety under the

guidance stipulated by Care Quality Commission.

 We understand, under the GDPR consent must be freely given and cannot be inferred from silence or pre-ticked boxes. At care package commencement, during our period of induction and assessment with the Service User we provide an active opt-in mechanism to prove that the data subject has confirmed consent in a manner and medium appropriate for the communication methods (including language) appropriate for the individual. Service Users can withdraw consent at any time via verbal or written notice, and Service Users are reminded of their right to do this, by support staff, on a regular basis. NHS-standards: As a team of NHS professionals all f our systems are aligned with the Information Governance Alliance: Records Management Code of Practice for Health and Social Care 2016 (RMCP). We work regularly with Clinical Commissioning Groups (CCG), which expect the services they commission to apply the RMCP. Our Information Governance Systems: Our information governance framework (IGF) at Eleven Sisters (ES) covers all records used for or with the care and treatment of our service users, staff records and administrative records likely to contain confidential information. All such records are handled and kept safely, securely and lawfully to the same standards established by the Data Protection Act 2018 – the general data protection regulations (GDPR) and the RMCP regardless of their formats, including written records, forms, photographs, audio-visual, CCTV records, computer and smart device electronic records. We manage any data, which we keep and to which we have access, so that the information is always held safely and securely and is lawfully used. In carrying on our business of providing care and treatment, ES will obtain and use the personal data of different groups of people: its service users and others relevant to them, its employees and others, such as contractors and suppliers of goods and services. ES is bound by law and its registration requirements to achieve established standards in its handling and management of information. Our IGF represents the systems, policies, procedures and processes adopted to ensure that data is always: 1. obtained fairly and lawfully 2. held securely and confidentially 3. recorded accurately and reliably 4. used effectively and ethically 5. shared and disclosed appropriately and lawfully 6. disposed of safely to the standards required, when no longer needed. 

The information governance framework includes several interrelated policies and procedures that contribute to its effectiveness.

 They include: 1. access to employees’ data 2. applications for access to a deceased service user’s care records 3. Caldicott principles 4. computer systems and internet: acceptable use 5. confidentiality of service users’ information 6. data protection 7. internet use: staff 8. internet use: service users 9. IT disposal 10. the use of mobile telephones 11. quality assurance: monitoring and reviewing the service provision 12. record keeping 13. sharing information with other providers 14. social media. We have procedures to ensure that records storage, arrangements for authorised access, information sharing, transfer of records, and quality of recording are all maintained to the required standard as per our respective information governance policies. ES will ensure that staff receive suitable training from induction onwards in the care service’s policies and procedures for safe handling and using information

 1. ensuring that all related policies and procedures on record keeping, confidentiality, consent, data protection are always adhered to by all staff, partners and stakeholders

 2. ensuring that all personal data in any form is kept safe and secure

 3. stating its commitment to continuously improving its information governance through its improvement plan.
